4Health Home Care Agency is the provider of high quality, personalised care for you in your own home.

Falling ill, returning from hospital, having an accident or coping with long term ailments are challenging enough, but in many cases you can avoid the additional stress of having to leave your own home. Employing an agency to care for you at home is a big decision and you need as much help and advice as possible. It is important to understand what services we provide. 4Health will do everything we can to help you decide what is right for you.

4Health can provide you with anything from companionship for an hour or so, to regular help with personal needs, cooking and supervision; or full time care if you have more complex requirements. This support can be delivered efficiently and cost effectively whilst at the same time ensuring respect for your dignity, privacy and safety.

We are committed to providing a high quality, efficient service to all of our patients. Your wishes are of the utmost importance and 4Health is here to help enable you to lead as full and active a life as possible.

 We will help you put together a Personal Care Plan, which will be tailored to meet your specific requirements and choices. Other health care professionals may have an input to ensure care is as comprehensive as possible. 4Health employ an experienced team of health care assistants and registered nurses who are chosen for their knowledge, skills and determination to put their patients first.

4Health are regulated by the States of Jersey and follow their Standards of Care. As we provide the highest standards, we have been selected for the States of Jersey Approved Provider Register.

Supported by two of Jersey’s leading GP practices at Castle Quay and Indigo Medical, 4Health will work with your own GP to make sure that you benefit from affordable, highly professional and compassionate care. 4Health will keep your information confidential and protect your privacy at all times.

Computerised patient records allow us, and your GP, to keep up to date with your care. If appropriate, and with your consent, your GP can view your progress which may prevent any unnecessary visits. Whilst 4Health has the support of Castle Quay and Indigo Medical Practices, there is no obligation on their patients to use 4Health and patients from other practices are also welcome.
